Providing physiotherapy to older people is a challenge, perhaps best described by the following: "Working with older people can present the physiotherapist with a set of challenges unparalleled in other areas of practice. The caseload is very mixed; patients with musculoskeletal, neurological, and cardiovascular problems may all be found in a single caseload and often in the same patient. In Tanzania there is no special hospital for servicing them, so as the ESBN planned to build special hospital which will be located in Tanga region.
Also, the ESBN plans to implement clown doctor therapy services. This will involve visits to the hospitals to help children's to adapts surroundings, they also distract from and demystify painful. Clown doctors use techniques such as magic, music, storytelling and other clowning skills to empower children’s and this will be done in Dodoma, Kilimanjaro, Mwanza, Dar es salaam and Arusha.
